where is category_index tables located?

This is the name of a table in the database CubeCart uses. To view the contents of thi stable, you will need to use an external utility, such as phpMyAdmin provided as a tool in your hosting control panel. The complete name of this table is CubeCart_category_index.

The physical location of the table is (probably) on the computer that is running the MySQL database server.

You will want to have phpMyAdmin dump the table CubeCart_category to a CSV or SQL file, then take that file and have phpMyAdmin import it into a different database.

A CSV file will have just the data and perhaps the top row being the column names, but nothing at all about the table structure. Use this to add more data to an existing, empty table.

A SQL file will have instruction that phpMyAdmin will use to completely reconstruct the database table, making sure that category_id values are maintained.

The table CubeCart_category_index is used to glue the category(ies) to the product(s). You do not want this table as it references products by product_id that will not be same in the second database.

what would i do for the store to open fine

At this installation, there appears to be a file named .htaccess, but (probably) PHP does not have permission to write the edited version to CubeCart's main folder.

I am also concerned that, based on the output you posted above, it shows the current content of that file followed by what CubeCart wants to add. The current content does contain the character sequence (called a string) seo_path, which is what is being tested for, so I do not know why CubeCart is complaining that it is missing.

For now, please make sure that PHP has permission to write to the folder where CubeCart is installed.
