How to add 'contact us' box if there is index.php only index.

I have found a post with a simple contact us form, here is the link:

The problem is, I do not have index.php file, when I try to open/edit it, it just says <?php die("Access Denied"); ?> - I think it means that this file does not exist anymore, as I have index.tpl but this is only my opinion.

Anyway, I followed all the steps in that thread except I have put the code in index.tpl , not index.php

now what it does when I enter my details into 'contact us' box, is that when I press 'send email' it loops back to my homepage. no 'thank you, your message has been sent' , and I did not receive the email either. so it does not work this way.

Any ideas how to add 'contact us' having index.tpl only?
