Shipping Module Configurations for "price per item"

I know 3rd party mod cant be discuss here but there isnt any response on my topic over at cubecartforums.org so i think i will ask over here.

I want my Shipping Rates to be as below:

1. $40.00 and below ($2 per item)

2. $40.00 and adove ($3 per item)

3. $100.00 and above (Free)

4. Self-collect (Free)

I just bought All In One Shipping Module CC4 v2.3 and i cant get my intended shipping fees to work with it.

How do i set it up such that if a person get 2 items of $40 and below the shipping fee will be $4 ($2 per item)?

e.g 2 x $46 will be $6 shipping rate ($3 per item), 3 x $46 will be Free shipping ($100.00 and above).

I cant seem to get it to work. Any help even if not using the above mod will be greatly appreciated.
